How to Handle Schedule X (Narcotic) Drugs in Your Pharmacy
Schedule X is the most tightly regulated category of medicines in Indian pharmacy practice. These are narcotic, psychotropic, and habit-forming substances with high abuse potential. Selling, storing, or dispensing Schedule X drugs without the proper licence, in the wrong quantities, or without maintaining the required register is a criminal offence under both the Drugs and Cosmetics Act and the Narcotic Drugs and Psychotropic Substances (NDPS) Act, 1985.
What Medicines Fall Under Schedule X?

Licence Requirements
To stock and sell Schedule X drugs, your pharmacy needs a specific endorsement on your drug licence for Schedule X substances. This is not automatic — you must apply separately. Requirements include:
- Separate locked steel almirah / safe exclusively for Schedule X drugs
- Adequate security to prevent theft or pilferage
- A qualified pharmacist on premises at all times Schedule X drugs are being sold
Register Requirements
You must maintain a dedicated Schedule X Register with the following for every transaction:
- Date of purchase / sale
- Name and strength of the drug
- Batch number and expiry date
- Quantity received / dispensed
- Patient name, address, and prescription details (for sales)
- Supplier name and invoice number (for purchases)
- Running balance after each entry
Critical: The Schedule X Register must be maintained in a bound book with numbered pages. Loose sheets are not acceptable. The register must be preserved for 2 years and produced immediately on Drug Inspector demand.
Prescription Requirements for Schedule X
- Prescription must be in duplicate (two copies)
- One copy retained by pharmacy, one returned to patient stamped "Dispensed"
- Prescription must carry doctor's registration number and full address
- Quantity must be explicitly written in words and figures
- No refills unless the prescription explicitly permits
Codeine cough syrups: Many states have additional restrictions on codeine — some states have banned retail sale of codeine syrups entirely due to widespread misuse. Check your State Drug Controller's current orders before stocking these.
